Request access, correction, deletion, or restriction of your data
Use this form if you want a copy of your data, need inaccurate information corrected, want consent withdrawn, or want us to review a deletion request. The Aroviah privacy team will review and respond using the contact details you provide.
What you can ask for
- Access to the personal data we currently hold about you
- Correction of inaccurate or incomplete information
- Deletion or anonymization, where retention is no longer required
- Withdrawal of consent for future processing of medical or coordination data
- Restriction or objection where applicable under relevant privacy laws
Before you submit
Include enough detail for us to identify your account, enquiry, or case. If treatment coordination is still active, some records may need to be retained for legal, medical, audit, or continuity reasons even when a deletion request is made.
